Patient Info Form

Please bring your pet, x-rays, medications and any current lab work with you to your initial surgery consult appointment.  We may ask you to not feed your pet the morning of your appointment.  It is fine to give them water.

All surgery patients will be confined to kennel or small room rest following surgery for 4-8 weeks depending on the procedure.  You will be given specific instructions regarding your pet’s medication, activity restrictions and physical therapy when you pick your pet up from the hospital.  Follow up visits for suture removal at 10 to 14 days post-operative and for x-ray recheck at 2 to 6 weeks post-operative may be required.  Suture removal is complimentary; however there will be a charge for follow-up x-rays, except on TPLO rechecks.

After your initial consultation, you will be given a treatment plan and an estimate of the charges.  We request payment when your pet is admitted for surgery.  We accept all major credit cards, however cash and personal checks are not accepted. If you prefer to pay in cash, please obtain a cashier’s check or money order.  In addition, we offer Care Credit financing for a convenient six-month term at 0% or with interest for 24, 36 or 48 months terms. You may apply for Care Credit online or by phone prior to your pet’s surgery appointment.
